Larry Dale Coach of the Year Nomination Form

Every District member, quartet, or chapter can suggest the name(s) of one or more individuals who have fulfilled the ideals that Larry Dale epitomized. The nominee must be a member of the Ontario District (regular chapter or Frank Thorne). Submit the name, a short resume, and the reasons why they deserve the award by March 15th.
